Required forms for Authors of Accepted Papers
Once your paper has been accepted, the editor will send you the instructions for submitting your paper in the final format.  At this time, you will also be required to submit the following forms:

• Copyright Assignment Form
• Color Order Form
• Offprint and Extra Pages Order Form
